Social Work Associate

• Conduct assessments, follow-up interventions and case management for clients with mental health conditions and/or dementia.
• Provide home visits, caregiver support and psychoeducation to clients and their caregivers.
• Plan and facilitate cognitive stimulation, reminiscence and other structured activities based on clients needs and interests.
• Support community mental health outreach, preventive programmes and public education initiatives.
• Monitor clients progress, provide appropriate support and escalate concerns to the Social Worker when required.
• Build partnerships with hospitals, Social Service Agencies (SSAs), grassroots organisations and community partners to strengthen mental health support and awareness.

Requirements

• Diploma in Social Work, Counselling, Psychology, Healthcare, Community Care or a related discipline.
• Preferably 2-3 years of experience in healthcare, community care or social services, particularly supporting mental health clients and caregivers.
• Strong interpersonal and communication skills, with good organisational skills and the ability to work independently and as part of a multidisciplinary team.
