We are hiring under Recruit Express headcount and you will be deployed to:
Contract: 6 Months (1 Oct 2026 - 1 Apr 2027)
Salary: $25-29/hour
Working Days: Monday-Friday
Shifts: 8am-5.30pm / 5.30am-6pm
Job Scope:
- Provide patient counselling and chronic disease management support.
- Conduct face-to-face, virtual and tele-consultations.
- Work with GPs to develop personalised care plans.
- Educate patients on diet, lifestyle, medication adherence and self-management.
- Conduct diabetic foot screening and spirometry when required.
- Coordinate patient care and referrals with GPs and Primary Care Coordinators.
- Maintain accurate clinical documentation and patient records.
- Support quality improvement projects and healthcare initiatives.
- Provide clinical guidance to Primary Care Coordinators when required.
Requirements:
- Minimum Advanced Diploma in Nursing.
- Registered with Singapore Nursing Board (SNB).
- Minimum 4-5 years of relevant nursing experience.
- Experience in patient counselling, chronic disease management, health education or community care preferred.
- Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is an advantage.
- Mental health management experience is an added advantage.
- Strong communication, counselling and care coordination skills.
- Willing to travel to various centres at north / central areas.
